Patients with end-stage kidney disease (ESKD) have a high rate of hospitalizations related to fluid overload and infections. #ArtificialIntelligence (AI)–driven models may improve patient care by predicting the risk of hospitalization.
Chaudhuri and colleagues conducted a retrospective, observational matched cohort study of adult patients with ESKD who were receiving value-based hemodialysis at integrated kidney care clinics across the United States in 2023. The results are now published in NEJM Catalyst Innovations in Care Delivery.
Two AI-powered machine learning models calculated risk scores (range: 0–1) and the models identified patients with a risk score of 0.64 or above who were at risk for hospitalization within 7 days in relation to infections or fluid status abnormalities. To prevent avoidable hospitalizations, case reviews and interventions were conducted for the patients identified by the models. The AI models generated scores for all patients, but only high-risk scores triggered case review and possible intervention.
The authors linked electronic medical records and Medicare claims data and conducted multivariate logistic regression analyses to examine the impact of AI-driven interventions on the odds of all-cause hospitalization in patients with ESKD. A total of 10,294 patients representing 83,928 risk scores were included in the analysis.
AI-driven interventions were associated with a reduction in the odds of hospitalization among patients with ESKD receiving managed kidney care. These findings underscore AI’s potential to assist health care providers with targeted risk interventions for patients with ESKD.

Encouraging whistleblowers to come forward to @rapeganginquiry is not just about gathering evidence — it is about restoring integrity to a system that failed to listen when it mattered most.
For too long, professionals, community members, and insiders who saw the warning signs felt silenced by fear: fear of losing their jobs, fear of being ostracised, fear of being labelled disloyal, racist, dramatic, or “difficult.” That culture of intimidation allowed abuse to flourish in the shadows. Breaking that silence is an act of courage — and an act of public service.
Whistleblowers are not traitors. They are guardians of truth.
Every email saved, every safeguarding concern logged, every meeting remembered, every decision questioned — these details matter. They create accountability. They expose patterns. They honour the experiences of victims who were ignored, dismissed, or blamed. When insiders speak honestly about what they witnessed — the missed opportunities, the institutional paralysis, the pressure to downplay — they help ensure that history cannot be rewritten.
This inquiry must be a place where whistleblowers are protected, respected, and believed. Anonymity must be honoured where needed. Emotional support must be available if required. No one should risk their livelihood for telling the truth — and yet, many have.
I would never ask anyone to do something that I wouldn't do myself........... As a whistleblower I have experienced the difficulties and risks that come with doing the right thing. Despite everything, I would do it again a million times over, as we all have a duty to protect our country's children by ensuring we have a system and work force in place which does just that.........

To those who know something and have stayed silent: your voice could be the missing piece. Your testimony could prevent another child from being harmed. Silence protects systems.
Speaking protects children.
Courage is contagious.
