A new law signed by President Donald Trump on February 5, 2026, allows Organ Procurement Organizations (OPOs) remote electronic access to the medical records of all POTENTIAL donors. “The health reform advocacy group, the Citizens’ Council for Health Freedom (CCHF), discovered the language on about the 900th page in HR 7148 and issued an ‘alert’ on January 27, 2026. ‘This provision will likely increase institutional focus on organ retrieval rather than patient privacy and family autonomy,’ the alert stated. ‘Further, the language could intensify pressure on families during end-of-life situations by automating electronic identification and referral of potential donors, eliminating a discussion between patients, families, nurses, and doctors about whether to notify an organ procurement organization.’” This means that patients with data shared through and across the Organ Procurement and Transplantation Network are the only patients within US healthcare that are not protected by federal privacy laws or security standards of any kind. @HeartlandInst @CCHFreedom https://heartland.org/publications/congress-gives-organ-procurement-organizations-faster-access-to-medical-records/

Unglaublich: Wieder findet heute ein bundesweiter „Aktionstag gegen Hass im Netz“ statt - deshalb gibt es mehrere Hausdurchsuchungen wegen Internet-Posts. Überwiegend geht es natürlich um den „Phänomenbereich Rechts.“ Das bedeutet: die Polizei durchsucht heute erneut bei zahlreichen Menschen die Wohnung, nicht weil es ermittlungstechnisch geboten wäre - sondern um damit ein Zeichen zu setzen. Mit rechtsstaatlichem Denken hat das nichts mehr zu tun. Language discordance in surgical care is a structural driver of inequity that affects patient safety, trust, and outcomes. Emerging interpreter technologies, including #ArtificialIntelligence (AI) and remote video interpretation (RVI), are rapidly entering clinical settings. However, implementation decisions are often made without understanding how patients themselves perceive these modalities or whether they view them as replacements or complementary tools within their care. To explore Spanish-speaking surgical patients’ perceptions of AI- and RVI-based interpreter technologies, and to understand how clinical context influences modality preferences, the author team conducted a descriptive concurrent mixed-methods study within a U.S. academic health system, enrolling 23 adult patients with Spanish language preference across the surgical continuum. The patients did not choose a single preferred modality; instead, they expressed context-dependent needs. AI was viewed as advantageous for its speed, privacy, and literal translation in straightforward or time-sensitive scenarios. RVI was favored for emotionally complex conversations and cultural nuance. Across narratives, patient agency emerged as a dominant theme. These findings support the development of a multifaceted language access infrastructure in which AI and remote human interpreters are deployed synergistically based on clinical sensitivity, urgency, and patient preference. I may be one of the luckiest people on earth. I made it to the age of 48 without death impacting me. 48 years without losing a sibling, a close friend, child, niece or nephew, college roommate or parent. I lay my dad to rest this week and am overwhelmed with gratitude that not only did I get 6 yrs to say goodby and that he got 2 years more life than the average American, but he was the first in my close circle to go. I don’t know anyone else that has been so blessed.

Als jemand, der täglich in Berlin unterwegs ist, muss ich sagen: Es wird von Jahr zu Jahr schlimmer. Mehr Dreck, der vor sich hingammelt. Mehr Schmutz, der keinen kümmert. Abfall liegt tagelang unter, vor und neben den Abfallkörben. Wild entsorgter Sperrmüll liegt wochenlang auf dem Trottoir. U-Bahn-Stationen bleiben jahrelang Baustellen. Immer mehr beschmierte Wände und Häuser, kaputte Menschen, Dinge, die einfach nicht funktionieren. Jeder einzelner Politiker von CDUSPDGRÜNENLINKEN, der hierfür Verantwortung trägt, hätte es verdient, vor die Stadttore getrieben zu werden. Damit er sein Glück, das das Unglück der Berliner ist, woanders suche. Es ist eine einzige kranke Zumutung, die sich in der deutschen Hauptstadt Normalität nennt. Jeder möge einen Bogen um sie machen. Man schämt sich nur noch.

Let me tell you what happened before I got to the OR this week. My patient needed surgery. I recommended it. My training, my board certification, and my years of clinical experience all pointed to one answer. The insurance company had a different one: not yet. That’s prior authorization and it is breaking American medicine. Here’s the number that should stop you cold: in a prospective multicenter study of orthopedic subspecialty practices across 6 states, 98.5% of prior authorization requests were ultimately approved when recommended by a fellowship-trained, board-certified orthopedic surgeon. 98.5%. The system isn’t catching bad medicine. It is a delay machine that consumes enormous resources, causes real patient harm, and exists primarily to make the economics of denial work in the insurer’s favor. The 2024 AMA survey of 1,000 physicians put numbers to what every physician already knows: - 39 prior authorizations per physician per week averaging 13 hours of physician and staff time - 93% of physicians say PA delays patient care - 82% say patients commonly abandon their recommended treatment because of PA - 29% report PA caused a serious adverse event — hospitalization, permanent damage, or death - 61% are concerned insurers are now using AI to systematically increase denial rates, with some tools producing denials up to 16 times higher This is not utilization management. This is obstruction at scale. Surgery is precision and focused on minimizing collateral damage. Prior authorization is brute force with disregard for the collateral damage. In my world, delay has a clinical cost. A patient waiting on PA for fracture repair, joint replacement, or spinal stabilization is losing function and sometimes losing the window for the best possible outcome. That cost never shows up on an insurer’s earnings call. Given that the average patient is on an insurer’s books go 3 years, delays can sometimes mean making someone else pay the bill for the necessary treatments. The consensus statement insurers signed in 2018, promising PA reform? Seven years later, only 16% of physicians working with UnitedHealthcare say anything has actually changed. They made another promise to Secretary Kennedy and Administrator Oz recently but is there any real change coming? Voluntary reform has failed. Legislative and regulatory action is the only path forward and physicians need to be loud enough for long enough that lawmakers act.
