Genau so war es: Merkel bereitete im Sommer 2020 die noch viel strikteren Maßnahmen vor, die ab Herbst das Land für über sechs Monate mit einem Lockdown lahmlegten. Im Sommer zwang sie zB andere europäische Länder, die ihre Maßnahmen schon ganz oder fast komplett beendet hatten, zurück auf Linie. Und das ergab besonders im Sommer gar keinen Sinn - außer man wusste: da muss noch was kommen. Darum sollte Drosten vor der angeblichen Sommerwelle warnen. Anschließend wurden Leopoldina und andere für die Rechtfertigung instruiert, Kritiker wurden kaltgestellt. Es ist einfach nur erschreckend, mit welcher kalten Berechnung, ohne jegliche Rücksicht auf Verluste und Kollateralschäden, hier vorgegangen wurde. Und die Mainstream-Medien spielten komplett mit.

The irony is that the genesis of many “real” diseases and disorders is, in fact, based on fraudulent science which then gets propagated throughout the literature and accepted as fact. When is all of that going to be called out as well? (Rhetorical question - never!)

My take on the critique offered in response to Ruuska 2026. I will take @GidMK's criticism as a linchpin. We heard from him that this is only a “tiny pool of patients and the care itself is no more or less harmful than many of the medical treatments we offer” and “if people paid this much attention to literally any other medical issue that children face we'd never talk about anything else” Yes, it is a tiny minority, but the treatments offered are far from “no more or less harmful” than others. And yes, “we”, those involved in drug development, do pay close attention to new treatment options for children. In fact, the FDA and EMA often force our hand: these days, your next blockbuster drug will not be registered unless you have agreed on a paediatric investigation plan. We go to great lengths and great expense to conduct such research even in diseases such as multiple sclerosis that occur extremely rarely in children and adolescents, much rarer that "gender dysphoria". On harm: the closest parallel to “gender dysphoria” is Body Integrity Identity Disorder (BIID), at least for those seeking amputation of healthy body parts. In BIID, it is widely frowned upon for a surgeon to remove such parts. The Ruuska 2026 paper describes a study that is as close to perfect as one can get when looking back 25 years. Healthcare records in many jurisdictions are routinely destroyed after a certain period. Everyone who has tried to investigate long-term outcomes knows the challenge this creates. Finland is a rare exception here (see table below). Therefore, one often has to sacrifice granularity of information for a broader but clinically meaningful endpoint, in this case, “attending specialist psychiatric care not related to gender care”. Basically, no one else can produce a comparable data set. Even the UK PATHWAYS trial will not deliver this information, because it will offer puberty blockers to everyone (the control group will only receive them 12 months later). In any case, the trial is currently on hold before recruitment started. Criticising the Ruuska 2026 paper is, of course, legitimate, as it is for any scientific paper. However, many critics appear to make a fundamental error: they seem to disregard the philosophy of risk–benefit assessment for medical interventions. The efficacy of an intervention must be established beyond reasonable doubt (typically via RCTs), while even tiny safety signals in the absence of clearly identifiable risk factors can render a treatment obsolete. Efficacy requires RCTs; safety signals can come from a wide range of data sources: no need for RCTs. For invasive medical transition, efficacy has never been established beyond reasonable doubt, whereas the Ruuska 2026 study delivers a clear safety signal in a robust data set. This is even more relevant because the Finnish centres adhered to the original Dutch protocol, offering invasive care only to patients perceived as mentally stable (apart from some mental reactivity directly linked to their dysphoria). This means their findings raise serious doubts about the ability of healthcare professionals (HCPs) to reliably identify risk factors for a newly emerging need for specialist psychiatric care. Yet such reliable identification would be a precondition for using the intervention in the face of suspected safety concerns. Personally, I believe that Ruuska 2026 settles the open questions: in the absence of an RCT on efficacy, the safety signals prevail. And I do not see an ethics committee approving any new such RCT after Ruuska 2026.
