Das Robert Koch-Institut (#RKI) hat die Anzahl der Risikogebiete für Deutschland mal wieder erhöht. Im Vergleich zu 2025 sind zwei hinzugekommen, die Gesamtzahl beläuft sich nun auf 185 Gebiete. Im Gegensatz dazu zählt die Weltgesundheitsorganisation (#WHO) hierzulande nur 55 Risikogebiete, vier weniger als im Vorjahr.
Ursächlich für die unterschiedliche Zählweise ist die Definition eines Risikogebietes, wie sie das RKI in seinem aktuellen Epidemiologischen Bulletin ausweist:
„Ein Kreis wird als FSME-Risikogebiet definiert, wenn die Anzahl der übermittelten FSME-Erkrankungen in mindestens einem der Fünfjahreszeiträume 2002–2006, 2003–2007 usw. bis 2021–2025 im Kreis ODER in der Kreisregion (bestehend aus dem betreffenden Kreis plus allen angrenzenden Kreisen) signifikant (p < 0,05) höher liegt als die bei einer Inzidenz von einer Erkrankung pro 100.000 Einwohner erwartete Fallzahl.“
Am Beispiel der Zahlen für Kinder und Jugendliche wird deutlich: Es gibt wenig Grund für Handlungsbedarf. Nur in 44 von 185 Risikogebieten, also weniger als einem Viertel der vom RKI ausgewiesenen Risikogebiete, gab es überhaupt Fälle von FSME in dieser Altersgruppe. In 33 dieser 44 Risikogebiete (75 %) trat wiederum nur ein einziger Fall auf.
Um ein vollständiges Bild der Pro- und Kontra-Argumente für eine evidenzbasierte Impfentscheidung zu erhalten, wurde der FSME-Fachbeitrag um die relevanten Argumente der STIKO ergänzt. Diese stellt die Kommission in ihren Epidemiologischen Bulletins dar.
Die erste Erwähnung der FSME-Impfempfehlung für Risikogruppen in Deutschland findet sich im Epidemiologischen Bulletin 15/98 von April 1998. Dort gab die STIKO aber keine wissenschaftliche Begründung ab, sondern nur die Indikation, wer wie geimpft werden soll. Eine wissenschaftlich, mit 2 DIN-A4-Seiten jedoch recht knappe Begründung lieferte die STIKO im Juli 1998 im Epidemiologischen Bulletin 27/98.
Spätere epidemiologische Bulletins (wie auch die zuletzt veröffentlichten 9/2025 und 9/2026) stellen vor allem die epidemiologische Situation dar und widmen sich der Aktualisierung der Risikogebiete – deren Zahl stetig ansteigt.
Außerdem lässt sich feststellen: Die #STIKO nimmt primär eine epidemiologische Situationsbeschreibung vor und beschränkt sich darauf, die relevanten Begrifflichkeiten wie „FSME-Kontrollgebiet“, „FSME-Endemiegebiet“ oder „FSME-Hochrisikogebiet“ zu erklären.
Dagegen vermeidet sie Angaben zur Wirksamkeit und Sicherheit der FSME-Impfung. Stattdessen entscheidet die STIKO lediglich auf Basis der Risikodefinition, dass „der Infektionsgefahr in diesen Risikogebieten durch Empfehlung entsprechender präventiver Maßnahmen für exponierte Personen Rechnung getragen werden“ müsse.
Es gibt weder eine systematische noch eine umfangreichere explorative Erörterung des Nutzens und der Risiken der FSME-Impfung durch die STIKO. Den Nutzen der Impfung „belegt“ die STIKO vor allem durch sehr vereinzelte Literaturangaben und die Angaben zum Impfstatus der übermittelten FSME-Fälle.
